Having a new baby can feel overwhelming at times, that’s why we make newborn healthcare simple. When your baby is born at Munroe Regional hospital, simply tell your obstetrician and the hospital staff that you have chosen Children’s Health of Ocala as your child’s pediatrician. Upon discharge, your hospitalist will let you know when to schedule your baby’s first visit to our office. Then, just call us to set up an appointment.
We welcome new patients to our family practice! For your first visit, please arrive at least 15 minutes prior to your scheduled appointment to complete all necessary paperwork. Children under 18 must be accompanied by a parent or legal guardian or have a letter signed by a parent or legal guardian giving permission to another adult to bring the child in for medical care.
Please bring the following to your first visit:
- Valid ID of parent or legal guardian
- Up-to-date immunization record
- Insurance card
- Previous medical records (if possible)
- Your new patient forms filled out
